Client Forms

The most important element of getting started is ensuring all of your paperwork is completed prior to your first appointment for consultation. Afterall, I am pretty sure you don't want to waste precious time during the thirty minute intake process filling out the form itself.  This page is filled with forms that are pertinent to your success in therapy and to you getting started with us.  

  Please take the time to print and look over them before filling them out. After you have had a chance to look them over, set some time aside to sit down and fill them in at a moment where you can reflect on the content requested and give a full view of your perception of that particular question.  Answer all of the questions, do not leave anything  blank unless you have questions to be addressed at the beginning of your session for consultation.  If all is understood and you have completed  filling out the forms, you can email them to us or simply bring them in with you on your first session.

This file contains a detailed overview of what Psychotherapy looks like and the constraints of privacy matters. It also contains your signature page. If you print your forms in advance, do not sign the consent unless you have a witness to sign it as well. We prefer you print and read over it, review it in detail during your first session prior to intake, and sign it in the presence of our staff.

Please be reminded that there is a document charge for this services as these requests take time away from normal business operations on the part of the therapist. The is a turn around time of 48 to 72 hours on all records requests. $45 for 48 hour and $20 for 72 hour service. Payments must be rendered before your request can begin processing. *There is no refund for this service*
